Having so many seniors around, Cherie says, is like having extra parents. She loves how they care about the staff, just as much as the staff cares about them. The residents are always willing to pass along some wonderful advice. At Evergreen Place in Streator, we are just one big family.

I am a Registered Nursing living in Streator. I was born and raised here, but also briefly lived in both California and Minnesota. I have been married to my wonderful husband for 35 years; we have two adult sons. I have been with Heritage Enterprises for 17 years. I worked at Heritage Health in Streator for 10 years as a staff nurse and then as the assistant director of nursing. I moved to Evergreen Place as the Wellness Coordinator seven years ago and became the director in 2012. My health care experience has always been with seniors. Before working at Heritage Enterprises, I worked at a county nursing home in Minnesota and on the extended care floor at St. Mary’s Hospital here in Streator. The best part of my job is getting to know, love and care for our seniors and their families.

Chrystal Middleton, Wellness Coordinator

I live just outside of Streator in a small rural community called Blackstone with my husband of 20 years, and our two daughters, ages 17 and 20. We share our home with two Dachshunds, a Yorkshire terrier, and our parrot. We also share our lives with our much-loved horses, donkeys, pigs, chickens, ducks, rabbits and cats.


My nursing career first began with Heritage Health when I worked as a CNA at age 18, while attending college. Since then, it’s been important to me to spend time participating in my family’s business that is agriculturally based, but I have worked within many facets of healthcare over the years as a registered nurse. I have served on many healthcare committees and teams while playing an active role in developing, changing, and implementing policies and procedures. I am glad to be back at Heritage-owned Evergreen Place, and I will work hard to ensure our leadership in providing high quality health care services to our residents while setting a standard of excellence within our community. The best part of my job as Wellness Coordinator here at Evergreen is seeing our seniors be active, fulfilled and smiling from day to day! Thank you for your trust in us as your partners in senior care!

Amy Spears, Campus Marketing and Community Relations Coordinator

I am a local Streator resident and feel blessed to have worked for Evergreen Place for over 6 years now. My job here is to educate the community and those seeking information about our senior care service line at Evergreen. You might see me in the newspaper or hear me on the local AM1250 WSPL radio station. I facilitate a Caregiver Support Group every 3rd Tuesday from 6-7pm at Evergreen and lead a Senior Resource Group in Peru, IL that focuses on bridging the gaps in care coordination across LaSalle County. I have become very passionate about what we do at Evergreen Place as it is all-encompassing and full of opportunity for the seniors we serve! Every senior has the right to know about one of Illinois’ smartest programs: Supportive Living. We work with all income levels so that every senior citizen can afford this amazing lifestyle!


My favorite part of my job is seeing residents of Evergreen flourish physically and socially in our beautiful environment. I also love knowing we can offer peace of mind to the family and friends of our residents. My name is Rhonda Freeman. I am the Life Enrichment Coordinator and have been working at Evergreen Place since December 2015.


I grew up in Dwight, Illinois. And upon graduating high school I moved to Colorado and opened my own business. I have two great kids, Austin 26 who still lives in Colorado and Maddison 21 who is a senior at Southern Illinois University majoring in Health Care Management and now tells me she would like to go through nursing training upon graduation.


My working background is in business management and sales. I have sold everything from auto glass in my own business to manufacturer beauty products, gourmet bird food and radio ads. I moved to Streator a few months ago because I became engaged to a long time Streator resident and great guy! And this brought me to Evergreen Place. I am really enjoying getting to know all of the residents. My favorite part of the job is discovering all of the residents’ many little talents. They all amaze me! Evergreen Place is a beautiful place for me to spend my time with smiling faces every day. We are having so much fun here at Evergreen Place!

I am Greg Schmitt, Building Services Coordinator at Evergreen Place in Streator. I have been with Evergreen Place for 7 years now. I have lived in Streator all of my life. My favorite part of my job is doing whatever I can for our residents. You find out that sometimes the smallest jobs or things you do for them means the most to them. My job has its days but most of the time it is rewarding.

I have been with Evergreen Place since July of 2009 and started in the dietary department. I was fortunate to be hired as the receptionist in 2011. My position includes answering the phones, filing paperwork, and maintaining schedules. I sometimes wear many hats and help out whoever needs some extra help! I am the first person you see when you come into our building, and always greet everyone with a smile!


I was born and raised in Streator, but currently live in a little farm town close by. I have been married to a great guy for 21 years and have three teenage sons!


The favorite part about my job is the residents; it is like having extra parents. They care about us, just as much as we care about them. The residents are always willing to pass along some wonderful advice. At Evergreen Place in Streator, we are just one BIG FAMILY!!
