When your loved one has Alzheimer’s or other cognitive impairments, the decision to move them from home to a community can be challenging. As professionals caring for individuals with Alzheimer’s disease and other forms of dementia, we understand the challenges that come with a move. At The Legacy: Memory Support, our experienced staff will work closely with the resident and his or her family to ensure a successful transition. Providing a safe, engaging environment that is not overstimulating is the key to success for individuals struggling with dementia.

While there are common characteristics, dementia affects each person differently; thus, through information gathering with the resident and those close to them, we personalize a plan for transition that best compliments the strengths of the resident and supports their cognitive challenges. The staff at The Legacy: Memory Support will work very closely with the resident and their loved ones to create meaningful and engaging days for each resident. This planning includes: 

  • Playing to the resident’s habits and strengths
  • Identifying activities that provide the resident enjoyment and a sense of purpose 
  • Personalizing their apartment to make it feel like home
  • Helping loved ones through the initial stages of transition; leaving their loved one in the care of our dedicated, compassionate, and qualified staff

We know that a transition from home to a community setting can be as challenging for the loved ones and caregivers as it is for the resident. We recommend that you have someone to support you during the initial transition period, especially on moving day. Feelings of grief and guilt, loss, and relief are all normal as you transition your loved one to full-time, community care.
